We are excited to announce that we are now offering the convenience of online registration through FamilyID
Family ID Registration Page (Click here for access)

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our sports programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile. You enter your information only once for each family member for multiple uses and multiple programs.

Please print this form along with the School Physical Sign off page. Upon Completion of the Sports Physical Evaluation please return the packet to the High School Main Office during summer hours from 8:00 am to 2:00 pm (Monday-Thursday).
Reminder that the school physician must review the packet prior to a student athlete's clearance. Physical packets should be handed into the High School 2 weeks prior to the start of the season.

***If your child has asthma, diabetes, a heart murmur, severe allergic or anaphylaxis or any other health concern, the links to the District’s medical forms are located on both the Family ID site and the School’s website under “School Nurse”. These forms must be filled out, signed by your physician and handed back into the school for clearance in order for your child to participate.

If your student plays sports or is interested in playing sports, you will find useful information here.
If your student has an existing medical issue requiring medical intervention, medication to be administered during school hours, or has asthma or any allergies, you will find the required medical forms here.

Physical Education is just as important as every other class your student engages in. If you student is unable to participate in gym due to an injury please provide a Dr's note indicating the type and length of restriction. More information here.
