Sports Physicals Information
If your student is interested in participating in school sports, it's important you read through the information on this page.

Physicals must be updated every school year.

Sports Physical Packets can be picked up in the High School and Middle School Nurses Offices.

Central Regional School District offers free annual sports physicals for all students at the end of the school year. Please check the school website for dates and details.

If you cannot participate in the school's yearly physical, and chose your own personal physician, the required Sports Physicals Packet must be turned in to the School Nurse's Office. These Physicals will then be sent out to be reviewed by our School Physician before your child can be approved for the sports season. This process takes additional time to process. Please be patient.

Important Dates
Printable Forms
If your child has any medical issues that require medical intervention or medication, there are required forms that must be filled out by your Physician or Specialist. These forms must also be updated every school year.
**** If you chose to have your physical done by your personal doctor, your physical packet must be turn in by the following dates*****
